Communicare Adult Day Health Care is a community-based adult day care program based in Sewell, NJ. Our programs are designed to meet the needs of the functionally impaired adult through an individual plan of care. Our focus is on educating and assisting participants and their families in adapting to the physical, mental, and emotional changes that are a natural part of the aging process.
We provide a safe, structured daytime program of health services and therapeutic activities for frail and medically needy adults. Staff works closely with family members to enhance the quality of life for their loved ones. Communication with families and caregivers is always a priority.
Adult Day Care Center
Communicare Adult Day Health Care's adult day care program provides a safe and supportive environment for adults with a variety of needs, including Alzheimer's disease, dementia, and other chronic conditions. The program offers a variety of activities and services to help clients maintain their cognitive function, social skills, and physical fitness.
Memory Care
Our memory care program at Communicare Adult Day Health Care provides adults with Alzheimer's disease and other forms of dementia. The program is staffed by experienced and qualified professionals who are trained to meet the unique needs of people with memory disabilities and support their need to maintain their cognitive function and quality of life.
Wound Care
Our comprehensive wound care program offered at Communicare Adult Day Health Care is set in place to help participants heal from a variety of wounds, including diabetic ulcers, pressure sores, and surgical wounds. The program is staffed by experienced and qualified professionals who are trained to provide the highest quality of care for wounds and ailments.
Lab & Pharmacy
Communicare Adult Day Health Care partners with a variety of lab and pharmacy services to help participants stay healthy and manage their chronic conditions. Our lab services allow participants to receive results of blood tests, urine tests, and other diagnostic tests. Lab tests can be ordered by a client's doctor, or can be requested by a client.
Nutrition
Communicare Adult Day Health Care provides nutrition services to help participants stay healthy and nourished. These services include meal preparation, dietary counseling, food education, special dietary needs. Communicare helps participants maintain their weight, manage chronic diseases, & reduce their risk of developing new health problems.
PT, OT & Speech Therapy
Communicare Adult Day Health Care provides a variety of physical therapy (PT), occupational therapy (OT), and speech therapy services to help participants improve their mobility, function, and communication skills. These services are provided by experienced and qualified therapists who are trained to meet the unique needs of each participant.
Mental Health
Communicare Adult Day Health Care offers a variety of mental health services to help participants manage their mental health conditions and improve their quality of life. These services are provided by experienced and qualified mental health professionals, including therapists, psychiatrists, and social workers.
